Motor Vehicle Accident (MVA) Claims

MVA/ Motor Vehicle Accident Claims: 

There are 3 types of MVA claims. 

  1. Insurer Claims in section A. We accept.
  2. Insurer Claims in Section B. We accept.
  3. Lawyer or Lien Claims for the Accident. We Do NOT accept.

How MVA Claims Work:

There are 2 sections of benefits for your MVA claim: Section A and Section B. 

Section A Claims:  

With Section A MVA claims, all treatments must be completed within 90 days of date of loss. Up to 21 treatments can be covered. Physiotherapy, Chiropractic, Massage and Acupuncture treatments are all included within this count. 

Once your 21 visits With Section A are completed any further visits will be billed to your extended insurance policy if you have one. (For example Alberta Blue Cross or SunLife.) The remaining balance not covered by your insurer and in addition to your current benefits, will then qualify for Section B Claims.

MVA Claims: Frequently Asked Questions:

Why do I have to use my group or private medical or dental benefits?

 As regulated by the Government of Alberta, automobile insurance requires that your private or group benefit plans, including health care spending accounts, be utilized first for any medical expenses not covered under the Diagnostic Treatment Protocol Regulation (DTPR). If there are approved treatments not covered by your medical plan, you should submit them to your adjuster for review.

 

What forms are needed to receive approval for treatments?

To process your claim, the Alberta Government mandates the completion of two forms: Notice of Loss and Proof of Claim (Form AB-1) and the Treatment Plan (Form AB-2). You can obtain these forms directly from your treating provider or by visiting the following link:

The Notice of Loss and Proof of Claim (Form AB-1) is to be filled out by you and enables communication with your Primary Health Care Practitioner.

The Treatment Plan (Form AB-2) is to be completed by your Primary Health Care Practitioner. Such as a Chiropractor, Physiotherapist, Physician, Dentist, Occupational therapist, or Psychologist.

Typically it is done by a Physiotherapist or Chiropractor who can best asses your injuries.

In the case where you have been in an accident and wish for us to claim directly to your insurer for services, we can help you with that.

We will require : 

  1. Your Adjuster information, Name and contact info. 
  2. The date of your Accident 
  3. The number of your claim

 In most cases we do not and cannot ever gain access to your personal plan information. Unlike dental and Pharma care it is very rare we can even do an eligibility check on your behalf. The only information we get is when we submit for payment. 

As such it is unlikely for us to tell you what your limits and plan conditions are before your appointment, or when your maximums have been reached. 

In some cases only a percentage of your appointment is covered or there are such conditions as requiring a Doctor's Note on file. We suggest contacting your insurer directly to fully understand your coverage before you come in.

However there may be limitations to your plan or restrictions based on territory or requirements.  Also if your plan information is incorrect, insufficient or the service provided isn’t covered by your insurer for any reason, you are responsible to pay for your appointment at the time of service.
